The Accident Investigation Bureau Releases First Report On Dana Airline Crash

The Accident Investigation Bureau (AIB) has released its first report on its investigation activities on last year’s crash of Dana flight 0992 in Lagos, with no conclusion regarding the cause of the incident.

The report contains technical information concerning the flight schedule, altitude of the flight, geographical circumstance of the spot of crash, flight crew and last conversation of the pilots regarding the sudden loss of both engines and failure of throttles.

The same information was made available last year in the preliminary report it issued few days after the crash and made available to a coroner making inquest on the crash in Lagos.

AIB said its investigative activities have included “visual examination of the aircraft wreckage, review of maintenance records and other historical information of the aircraft, documentation of the training and experience of the flight crew, determination the chronology of the flight, review of recorded data, reconstructing the aircraft refueling, and collection of related fuel samples, and interviews of relevant personnel”.

The investigation, AIB indicated, was only in compliance with standard recommendation of the International Civil Aviation Organization [ICAO], which requires it, as ‘State of Occurrence’, to institute the activity of enquiry into air accidents.

The report, according to AIB spokesperson Tunji Oketunbi, was also in fulfillment of a requirement of the International Civil Aviation Organization which mandates that the public must be informed of the progress of an investigation 365 days after an air accident.

Aviation Authorities Suspend Dana Airline's license; Data Recorder Found

Nigerian aviation authorities suspended Dana Air's operating license today as torrential rain hampered recovery efforts after the deadly weekend crash.

"As soon as we have concluded the recertification of the airline, if they are capable, then their license will be reinstated," said Sam Adurogboye, a spokesman for the Nigerian Civil Aviation Authority.

Authorities have recovered the flight data recorder and the cockpit voice recorder, which will help investigators piece together what caused Sunday's crash that authorities say killed at least 153 people aboard the plane and at least 10 people on the ground.

So far, rescuers have recovered 153 bodies, including that of a woman clutching a baby, from the crash site. But a downpour forced a suspension of recovery efforts Tuesday.

OMG! Dana Airline Official says they were forced to Fly the Faulty Plane

An official of Dana Airlines has confirmed to Channels Television that the ill-fated Dana Air flight 0992, MC Donnell Douglas (MD 83) was faulty shortly after it left Lagos and stopped over in Calabar.

She alleged that the Indian owners of the airline threw caution to the wind and insisted that the plane must fly in a bid to maximise profit, thereby sending the plane over to Abuja to pick passengers, when it should have being returned to Lagos for further repair.
She also claimed the flight has had persistent history of faults with its hydraulics in recent times and it was not supposed to have flown.

According to the official, “the plane has being giving faults for a very long time. There was a case when it was on ground in Uyo for over six hours, because of delayed flight, it had a bolt. And then in Abuja it happened a few days ago, then some people went with the aircraft but they could not come back, because it had a fault there and it couldnt leave Abuja.” “The same engineers that fixed it and then they sent crew to bring it with passengers to Lagos.”

Dana Plane Crash: Aviation Minister says Plane declared Emergency Landing + Photos of Victims

The Minister of Aviation, Mrs. Stella Oduah, who spoke at a press briefing at the Nigerian Civil Aviation Authority headquarters, Murtala Muhammed Airport Ikeja, Lagos, said the ill-fated aircraft coming from Abuja had declared emergency with the Lagos control tower at 3.43pm. A minute later, she said the aircraft disappeared from the control radar.

According to Oduah, the Federal Government will ensure that the accident is thoroughly investigated to determine the exact cause of the accident in order to prevent a similar occurrence in the future.

Meanwhile people have been posting photos of their loved ones involved in the crash on twitter and BBM... May their gentle souls rest in peace, so sad!
